Medical Home Initiatives Seminars
The trainings contains information regarding the state’s new Primary Care Case Management program – known as Illinois Health Connect – and, for those trainings in Cook, Madison, Perry, Randolph, St. Clair, and Washington Counties, information on the Illinois Client Enrollment Broker, including Illinois Health Connect, Harmony Health Plan, and Family Health Network. The goal of the training tour is to educate a diverse group of stakeholders, including healthcare and social service providers, case managers, community-based organizations and other healthcare advocates, all of whom will be instrumental in the implementation of the new medical home initiative.

What is a medical home?
A medical home is where patients go to see their primary care provider. A medical home—a solo or group practice, clinic, or health center—houses a patient’s medical records.
Who MUST enroll in the medical home initiative?
Most people with an Illinois Department of Healthcare and Family Services or All Kids medical card must join Illinois Health Connect.
Who CANNOT enroll in the medical home
initiative?
- People who receive Medicare
- Children under 21 who get Supplemental Security Income (SSI)
- Children in foster care and children who get subsidized guardianship or adoption assistance from the Department of Children and Family Services
- Children under 21 who are blind or who have a disability
- People who reside in nursing facilities
- American Indians and Alaska natives
- Refugees
- People who get home and community-based services such as the Community Care Program, the Home Service Program, or community services for persons with developmental disabilities
- People residing in Community Integrated Living Arrangements
- People enrolled in the following programs:
- Illinois Healthy Women
- All Kids Rebate and FamilyCare Rebate
- Illinois Cares Rx (SeniorCare/Circuit Breaker)
- Transitional Assistance (19 or older)
- Emergency Medical Only
- Hospice
- Sexual assault, renal, and hemophilia Programs
- People with good private health insurance
- Children under age 21 whose care is managed by the Division of Specialized Care for
- Children (DSCC) of the University of Illinois at Chicago
What is Illinois Health Connect?
Illinois Health Connect helps most patients receiving medical public assistance obtain a medical home for all of their medical needs. Illinois Health Connect will serve as the sole medical network for most Medicaid patients outside of Cook, Madison, Perry, Randolph, St. Clair, and Washington Counties.

What is the Illinois Client Enrollment Broker?
The Illinois Client Enrollment Broker is the unbiased enrollment broker, which manages the medical home initiative in the following six counties: Cook, Madison, Perry, Randolph, St. Clair, and Washington Counties. Most Medicaid patients in these six counties are required to choose a health plan - Illinois Health Connect, Harmony Health Plan, or Family Health Network (FHN only available in Cook County), in addition to a Primary Care Provider; and, ICEB assists patients in their decision-making process.
